The rules for using Sorta. Plain English, no surprises.

1. Who these terms apply to
These terms apply to clinics, practices, and healthcare providers ("you" or "Clinic") that use Sorta's patient intake automation platform. By signing up for Sorta, you agree to these terms.
2. What Sorta provides
Sorta provides a software platform that automates patient intake paperwork for outpatient clinics. Specifically:
- Patients fill out your existing forms on their phone before appointments
- That data auto-populates across every form your clinic uses
- Clean PDFs identical to your originals are generated automatically
- Sorta works on top of your existing EHR — we don't replace anything
3. Your responsibilities
As a clinic using Sorta, you agree to:
- Use Sorta only for lawful healthcare administration purposes
- Maintain the confidentiality of your login credentials
- Ensure patients are informed that their intake forms are collected digitally
- Execute a Business Associate Agreement with Sorta before processing real patient data
- Comply with HIPAA and all applicable healthcare privacy laws
- Not share login credentials between staff members — each staff member should have their own account
- Not attempt to access another clinic's data
4. Patient data
You are the Covered Entity under HIPAA. Sorta is your Business Associate. Patient data entered into Sorta belongs to your clinic and your patients — not to Sorta. We process it only to provide you the service. We will never use your patient data for our own purposes, sell it, or disclose it except as required to provide the service or as required by law.
5. Pricing and payment
Sorta charges $300 per month per clinic location after the free 30-day pilot period. Payment is due monthly. You may cancel at any time — no cancellation fees, no long-term contracts. Your data remains accessible for 30 days after cancellation. After 30 days your data is permanently deleted.
6. Free pilot
New clinics receive 30 days of free access. No credit card required to start. At the end of 30 days you can choose to subscribe at $300/month or cancel. We won't charge you automatically — we'll reach out to discuss continuing.
7. Uptime and availability
We aim for 99.5% uptime. Sorta is hosted on Microsoft Azure which has industry-leading reliability. Planned maintenance will be communicated at least 24 hours in advance. We are not liable for downtime caused by factors outside our control including Azure outages, internet disruptions, or force majeure events.
8. Intellectual property
Sorta owns the platform, the software, and the underlying technology. You own your clinic's forms, templates, and patient data. We don't claim any rights to your content.
9. Limitation of liability
Sorta is not liable for indirect, incidental, or consequential damages. Our maximum liability to you is limited to the amount you paid us in the three months preceding the claim. We are not a medical provider and Sorta does not provide medical advice.
10. Termination
Either party may terminate the relationship at any time. We may suspend or terminate accounts that violate these terms. You may cancel your subscription at any time from your account settings or by emailing us.
11. Changes to these terms
We may update these terms. We'll notify you by email at least 30 days before material changes take effect. Continued use after that date constitutes acceptance.
